General Transcript Notation Information

40 hours are required for Transcript Notation (TN).

All of the internships have been posted on Aggie Job Link. Interns should submit TN's by the end of the quarter. Below are detailed instructions to submitting TN.

Instructions for Submitting Transcript Notation

Creating Your Proposal (To be done within the first weeks of your internship)
1.Log on to Aggie Job Link or Create a new account if you don’t have one
yet (see left margin of iccweb.ucdavis.edu)
2.Click “Transcript Notation” under the “Quick Links”
3.Click “Add new” to start a new form
4.Select Organization
For VMTH internships, type “Veterinary Medical Teaching Hospital”
For CLAS internships, type “Campus Veterinary Services”
5.Your internship should appear in the drop down menu
6.Fill out other contact info, etc
7.Time Commitment- click and enter the total number of hours you
completed, a detailed day by day time sheet is not required unless
requested by your supervisor (there’s a link to a printable time sheet if
you need it)
8.Click “save” and this will save your proposal in the system and allow
your name to show up on your supervisor’s list of interns to evaluate

Submitting Your Final Report (To be done within the last weeks of your internship)
1.There is a tab at the top of the page labeled ‘Student Final Report”. The
tab only shows up after you have saved your proposal.
2. Have your essay prepared in the word processing program of your choice
and upload it.

Your essay will be used by the ICC to monitor the effectiveness of
internship services and to determine how well the learning objectives
associated with this experience were met. Your candid, thoughtful
evaluation will assist the ICC staff in that appraisal and provide valuable
feedback.

Unsatisfactory essays will be returned for additional work and
resubmission, and may result in denial of transcript notation for your
internship.

Format: No more than 5 pages; double spaced; 1" margins; 12 pt font.

Questions to answer in your essay:
•What significant activities, assignments, and/or projects did you work on?
•What specific skills and abilities did you apply and/or acquire through
your internship?
•What knowledge did you gain about the internship organization and its
structure?
•Did the internship meet your expectations and assist you in setting goals
for the future? Why or why not?
•Did the internship influence your future academic and career plans? If so,
how?
•What were your internship supervisor's expectations? Did you meet them?

If this is your 2nd or 3rd quarter in the same internship, your essay
should describe any new skills, activities or assignments. Give examples of
how you have continued to grow and develop as a result of being in the same
internship for multiple quarters. You must submit a different, original
essay at the end of each quarter. If you submit a duplicate essay, you may
be denied transcript notation for your internship.

3. Select your internship title

4. Clicking “save” will result in submitting your essay and completing the
Transcript Notation Process.  You will be awarded notation upon review by
your supervisor and ICC coordinator.
